Proof of Life
If you need to establish proof of life and give instructions for Social Security checks to be sent to an authorized party, please follow these instructions:
1-Draft a letter using the following format (click here)
Please indicate where your check should be forwarded:
For example:
Bank in Panama: indicate the name of the Bank.
2-If you can’t come to the Consulate in person, you will need to sign the letter before a Notary Public.
3-The original letter has to then be mailed to our offices along with a pre-paid envelope, a copy of your cedula, and your social security card.
4-Once the letter has been received, the authentication process may take from 10-12 business days.

THERE IS NO FEE FOR THIS SERVICE.
THE DOCUMENT OF PROOF OF LIFE SHOULD BE IN SPANISH AND HAS TO BE RENOVATED EVERY 6 MONTHS.
